The Health and Well-Being Program for Asian Families is funded by Contra Costa County’s Mental Health Department through the Prevention and Early Intervention Planning. The program is designed to serve the Asian communities throughout the entire Contra Costa County. The goals of the program are to use culturally appropriate and best practices to strengthen communities to increase wellness and reduce stress and isolation, to decrease the likelihood of needing service of many types.
The program has two case managers that are linguistically and ethnically representatives of the program’s targeted Asian community members. The program uses a scientifically proven curriculum, the Strengthening Families Program, to provide the following services.
