Our delivery model offers best practice, client-centered, tailored, integrated case management solutions of care interventions—various types, scale, and intensity of care to create sustainable outcomes for those with complex barriers and needs (CalWORKs, GA, Disability SSI, AB109 Reentry, Unhoused and Homeless, DV Survivors, Young Adults, and Refugees).

THE LAO FAMILY CARE INTEGRATED CASE MANAGEMENT MODEL FOR SELF-SUFFICIENCY

Lao Family strives to meet the client’s immediate, long-term and unique needs. The client-centered approach engages individuals and families to participate to achieve their self-sufficiency and stability goals. • Housing for all populations, including unhoused adults, transitional-aged youth, single parents, seniors, veterans, formerly incarcerated adults
• Career Technical Education Certificate Offerings from various local partners (Class A truck, Class B commercial, and school bus training, asbestos/lead/mold/confined space/construction environmental remediation management training, digital literacy and IT system administration, cyber security, certified nursing assistant and other non-traditional healthcare fields), working waterfront training sector—passenger ferries, commercial vessels and military vessels.
