Built on Community, Access, Resources, and Education, the CARE Model provides a clear, unified pathway for individuals and families to achieve wellness, stability, and long-term opportunity.
CARE is delivered through two major service hubs:
CARE Campuses (Housing + Holistic Support)
CARE Community Centers (Education + Workforce + Empowerment)

CARE CAMPUSES
Housing + Healing + Pathways to Independence
CARE Campuses are transitional housing communities where participants live onsite while receiving holistic, wraparound support designed to help them rebuild their lives.

- CalAIM-informed health & behavioral health support
- Life skills, health education & wellness coaching
- Workforce development & IPS employment services
- Digital literacy & education opportunities

CARE Community Centers are non-residential hubs designed to bring essential education, technology, and workforce opportunities directly into neighborhoods.
These centers strengthen the “Human Services” pillar of LFCD’s mission by helping participants build the skills and confidence needed for long-term success.
