Lao Family Community Development is proud to announce the expansion of our services into Yolo County. Through our partnership with Partnership HealthPlan of California, we now offer CalAIM Enhanced Care Management (ECM) and Community Supports (CS) to eligible Medi-Cal members throughout the region.
Our new West Sacramento site serves as a centralized location for culturally responsive care coordination, housing stabilization services, and whole-person support to individuals and families navigating complex health and social needs.
Lao Family remains committed to expanding access, improving outcomes, and empowering communities across Northern California.
